Personal Mortgage Information Planner

  • Initial Information
    • Home address
    • Mailing address
    • Home phone number
    • Cell phone number
    • Email address
    • Birthdate
    • Social security number
    • Driver’s license
    • Resident Visa (if not a US citizen)

  • Housing History
    • 24 months of home addresses
    • 12 months of landlord’s name, address, and phone number
    • Final paperwork from previous refinancing or purchase of home
    • Current home loan statement

  • Job History
    • Employer’s name, address and phone number (last 2 years)
    • Beginning dates for each employment (last 2 years)
    • Reason for any job gaps (last 2 years)
    • Certificate of Eligibility and DD 214 (for VA loans)

  • Income Documentation
    • Current pay stubs
    • W2s (last 2 years)
    • Tax returns (last 2 years-for self-employed, retirees and real estate investors)
    • Current year’s profit and loss statement (for self-employed)
    • Divorce decree and child support plan if receiving payment

  • Assets
    • Bank statements (last 2 months)
    • Investment account balances
    • Rental agreements and monthly statements (for real estate investments)
    • Anticipated amount of gift from a relative

  • Liabilities
    • Divorce decree and child support plan if making payments
    • Bankruptcy documents with discharge page
    • Monthly payments (such as credit card payments, car, motorcycle, RV and boat loans, student loans and personal bank loans)
